We begin with a comprehensive assessment covering nutritional status (including lab work where indicated), physical mobility, mental wellness, and lifestyle habits. Sleep patterns, social activity, and stress levels are all considered. The result is a 360-degree picture of your parent's wellbeing.
Personalised Wellness Plan
Based on the assessment, our dietitian, wellness instructor, and care manager collaborate to design an integrated wellness plan. It includes specific dietary targets, a weekly exercise schedule, mindfulness sessions, and clear wellness goals for the next three months.
Ongoing Support & Progress Review
The plan is only as good as its execution. Our team provides hands-on support, tracks adherence, and reviews progress monthly, adjusting the plan based on what's working, what isn't, and how your parent's health evolves. You receive a monthly wellness report.
